IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

The IRS has made changes to Form 1095-C reporting requirements. As a reminder, Form 1095-C is the form that reports your eligibility and enrollment in an employer-sponsored medical plan to the IRS. While the form is not required when filing your taxes, it would be required if you were ever audited.

Beginning this year, if you would like to receive a copy of your Form 1095-C, you will need to make a formal request. You will no longer automatically receive a copy mailed to your home address. To request your 1095-C form, complete a 1095 Copy Request and Delivery Consent Form and send it to BSC Case System: https://uncsys.service-now.com/uba.
